Concave profile? Anterior maxillary distraction
It is always the goal of the doctor to provide the treatment which has the least negative effects on the patient and maximum benefit and is minimally morbid. Our patient was a 25yr old female who was dissatisfied with her appearance and complained of what she called a ‘Concave face’. It was because she suffered from a previously treated cleft lip and palate deformity which usually inhibits the unhindered growth of the upper jaw and hence gives that flattened appearance. At the Richardsons dental and craniofacial centre we evaluated her CBCT scan and diagnosed her upper jaw ie.maxilla as being deficient in dimensions.Dr.Sunil advised to perform an Anterior maxillary distraction procedure for her which has overtaken all previous methods of treating these problems due to its reliability. With the great success stories of all patients who have undergone this procedure; the same was performed for this patient as well. Under general anaesthesia, the anterior part of upper jaw was cut and mobilised and suspended by a pre-fabricated screw appliance. This screw used is of German make and being the best of its kind keeps per-procedure failures negligible. After a resting phase of 6 days, the screw was turned by four turns twice a day for about 6 days and a total of 8.64mm of new bone and forward movement of the maxilla was achieved. This phase of treatment did not need any hospitalisation and was almost a pain-free procedure. After the desired movement was completed the new position of the upper jaw was retained by fixing a prosthetic tooth in the newly created gap. The new bone formed in the gap is just like any other bone of our body. The profile of the patient has drastically changed and she carries no stigma of being a cleft lip and palate patient.
