Root Canal Treatment or RCT is the removal of pulp tissue from the tooth. A tooth consists of hard enamel, dentin, and cementum with soft pulp tissue in the center. The pulp is the living tissue in a tooth. Pulp tissue consists of blood vessels, nerve fibers, lymphatics, and connective tissue. Pulp forms the major route of nourishment to the tooth. Pulp tissue extends all the way from the crown as a pulp chamber to the root as a root canal. A number of canals in a tooth varies from tooth to tooth. Decay or dental caries are visibly seen as blackish discoloration on the tooth surface. Cariogenic bacteria present in the oral cavity acts on the tooth surface results in caries or cavities. In the initial stages of dental caries, the organisms are active in enamel, the hardest part representing mere blackish discoloration. Once the decay spreads to dentin with numerous nerve endings, mild to moderate dentinal sensitivity is felt. As the decay approaches the pulp, the pain of varying intensity is experienced. Subsequently if untreated in this stage, the infection spreads through the canals to the root thereby into the underlying bone representing a swelling or pus discharge through sinus opening.
Root canal treatment is given as a treatment option to avoid the extraction of a carious painful tooth. Root canal treatments are suggested as the last sort of resort for severe sensitivity. Root canal treatments are also done intentionally before certain dental treatments like fixed partial dentures for teeth replacement. They are also indicated in a fractured tooth with pulp exposure.
Root canal treatment involves complete removal of the pulp tissue, thereby devitalizing the tooth. Devitalization eliminates only the proprioception of the tooth, its function in mastication and speech is preserved with the help of RCT.
Root canal treatments are usually done under local anesthesia to reduce pain and anxiety in patients. Root canals are accessed through the openings made on the occlusal surface of the tooth. The canals are vigorously cleaned and irrigated to make them free-off infections and irritants. Canals are treated with intracanal medicaments. Canals are obturated permanently with gutta-percha. They are sealed with permanent filling material, usually with a miracle mix in posteriors and glass ionomer cement in anterior. During this treatment process patients are under mandatory antibiotics.
